APTSM welcomes new referrals and wishes to make your experience as easy a process as possible. After receiving your referral from a Physician call either the or the to schedule a time to begin treatment at your convenience.

 Below are links to the (4) forms that may be printed, read and filled out at your leisure. Bring these and your insurance information, as well as the Physician's prescription to your first treatment. The Patients Account Representative will verify your insurance information.
