APTSM welcomes new referrals and wishes to make your experience as easy a process as possible. After recieving your referral from a Physician call either the Oil Center Location or the North Location to schedule a time to begin treatment at your convienence.

Below are links to the (3) forms that may be printed, read and filled out at your leisure. Bring these and your insurance information, as well as the Physician's prescription to your first treatment. The Patients Account Representative will verify your insurance information.
